This form is to be completed upon discovery of vulnerabilities that may lead to a data security incident(s) or following any identified occurrence of a data security incident(s). It may also be used to inquire about ongoing or recurring events that resulted in data security incident(s).

Data Security Incident Reporting (Temporary Notice)

We are currently updating our incident reporting system. Until the new secure form is live, please report all data security incidents or potential vulnerabilities via email.

This includes:

  • Suspected data breaches
  • Unauthorized access attempts
  • Security vulnerabilities
  • Suspicious system or account activity
  • Recurring or ongoing security events

When reporting, please provide:

  • Brief summary of the incident
  • Systems or data involve
  • Date and time discovered
  • Impact observed (if known)
  • Any logs, screenshots, or evidence

Our Cybersecurity Team will review submissions promptly and follow up when required.

NOTE: Please do not include passwords, payment information, or highly sensitive personal data in emails.
